    “Ruptura de útero didelfo en el segundo trimestre de gestación”

    Müllerian malformations are a group of not very frequent anomalies caused by a detention of its development during embryogenesis. The Didelphys Uterus belongs to one of them, and it occurs when both paramesonephric ducts fail to merge correctly. And causes the formation of two separate uterine bodies with double cervix. This entity can maintain normal pregnancy evolution, however it also generates obstetric complications that put maternal and fetal well-being at risk. We present the case of a 28-year-old patient from the center of the Interandean Region of Ecuador, with no relevant medical history, but history of a previous cesarean section. At the moment going through her second pregnancy, who presented with a sudden alteration in the level of consciousness and abdominal pain, so she was transferred to a private hospital, admitted with unfavorable vital signs, ultrasound is performed and an abdominal pregnancy of 16 weeks of gestation, absence of fetal heartbeat frequency so was transferred to Hospital General Docente Ambato, hemodynamically unstable, it is decided admission to the Obstetric Center where hematic volume was compensated and she passed to the operating room for emergency laparotomy. The findings were: Evidence of Uterus Didelphys with rupture of the left hemi-uterus, hemoperitoneum, placental accretion and dead fetus of approximately 16 weeks of gestation. Left Subtotal hysterectomy and left total salpingectomy was performed. After the procedure, the patient was admitted in hospitalization area where she remained stable with favorable evolution and recovered without complications.
