    Adherencia terapéutica y apoyo familiar del paciente con enfermedad renal crónica

    Introduction: Chronic Kidney Disease is characterized by being a pathology that affects the functionality of the kidneys, preventing them from fulfilling the function of filtering the blood adequately, strictly necessary drugs must be used, in adequate doses and intervals, during the necessary time, patients dependent on care have difficulties in adapting to their therapeutic treatment, becoming a public health problem. Objective: To analyze therapeutic adherence and family support of patients with Chronic Kidney Disease. Method: The research is mixed, cross-sectional. In the qualitative phase, the information was collected through semi-structured interviews with 10 family members, at which time the information was saturated, and the quantitative phase through the MoriskyGreen Test, applied to 36 patients in the hemodialysis area of the Ambato General Teaching Hospital. Results: It was evidenced that 52.8% of the patients present adherence to the treatment while 47.2% of the patients presented hypocompliance, in relation to the management by the relatives, it was evidenced that they try to comply with the established indications, however, when administering the medication they are mixing it, which can generate drug interactions. Conclusions: Therapeutic adherence is related to family support to fully comply with the treatment, therefore, help and support measures are implemented so that they can lead their lives without complications

    Afrontamiento del covid-19 en el personal auxiliar de enfermería

    Introduction: The global crisis due to COVID-19, has produced changes in health institutions, affecting the mental health of health personnel, so it has been necessary to implement coping strategies to adapt to the situation. Objective: To identify COVID-19 coping in auxiliary nursing staff. Methodology: This is quantitative research of non experimental design, cross-sectional, with a descriptive scope. The study population was 100 nursing assistants of the Hospital General Docente Ambato. The information was collected through a survey with 21 closed questions, divided into 3 dimensions: adaptation or assimilation, adequate management of emotions and social limitations. Results: The coping strategies used were problem solving with 55%, worry solving with 47.5% and resistance to difficulties with 56%; the nursing assistants presented a medium level of coping with a percentage of 41%. Conclusions: COVID-19 coping has been progressively improving, healthcare professionals have changed their way of thinking and acting in the face of illness, allowing their work development, for such reason, it is essential that coping strategies continue to be applied and encouraged among workers, in order to maintain their health both physically and psychologically

    Aplicación del lenguaje estandarizado en el desarrollo de la práctica pre-profesional

    Introduction: The standardized nursing language is a tool that integrates theoretical frameworks to identify problems, interventions, and results. Objective: To determine the application of the standardized language in the development of the pre-professional practice of the Nursing Rotating Interns of the Technical University of Ambato. Methodology: Quantitative approach with a non-experimental, cross-sectional design with a descriptive scope. For which, a census sample of 110 participants was addressed using a questionnaire designed and validated with a Cronbach's Alpha of 0.94, guaranteeing the reliability of the instrument. Results: The applicability of the standardized language on a daily basis comprises 29.8%, 91.8% of the participants report that this methodology improves their cognitive abilities; while 90.9% expressed the usefulness of applying the case study format; 73% recognized that this tool improves the quality ofpatient care and generates satisfaction when using it; to 40.4%, it provided them with positive experiences. Conclusion: The application of standardized language during preprofessional training encourages the continuous improvement ofthe skills of future professionals to develop their activities in the hospital and community area based on scientific evidence.

    ATENCIÓN DE ENFERMERÍA EN PACIENTES ONCOLÓGICOS CON CUIDADOS PALIATIVOS

    Introduction: Cancer continues to be a problem in health care, especially in cancer patients with terminal diagnoses, the execution of palliative care is aimed at improving the quality of life by providing a dignified death. Objective: Describe nursing care in cancer patients with palliative care. Methodology: Systematic review in selected studies or investigations through the application of the PRISMA method. Results: Selection and analysis of 16 original and review articles that meet the selection criteria under reliable supports. Discussion: The number of selected articles supports the importance of nursing care in cancer patients, describes the symptoms present regardless of the type of cancer the patient presents, and also explains the degree of patient satisfaction with the interventions carried out. by nursing. Conclusions: Palliative care has become a fundamental point in the care of patients with oncological diseases, nursing has a fundamental role where their care is focused on physical, psychological and emotional well-being to offer comfort to both the patient and their family until the end of the day. end of life Palliative care focuses on the integrality of the patient's quality of life.

    CALIDAD DE ATENCION PERCIBIDA EN PACIENTES CON HIPERTENSIÓN ARTERIAL EN ATENCIÓN PRIMARIA EN SALUD

    Background: the study analyzed the quality of care perceived by patients with high blood pressure in a health center in Ecuador, considering dimensions such as accessibility, opportunity, continuity, comprehensiveness and cultural competence of the services. Hypertension in older adults represents a public health problem in the region; (2) Methods: quantitative, transversal and descriptive design. The sample was 40 patients over 65 years of age with HTN. A validated 10-item questionnaire was applied on perceived quality in several dimensions: accessibility, opportunity, continuity, comprehensiveness and cultural competence; (3) Results: limitations were evident in the capacity to respond to urgent needs, lack of continuity in care by the same professional, deficiencies in mental health counseling and moderate dissatisfaction with services; (4) Conclusions: there are significant gaps in the quality of primary care for hypertensive patients regarding accessibility, opportunity, comprehensiveness and cultural competence. Improvements are required in these areas to ensure more responsive and patient-centered care.

    Calidad de la práctica formativa en médico-quirúrgico para el desempeño profesional de enfermería

    Background: In Nursing science, the evolution of instruction performs is an essential pillar to assurance better skillful implementation. Demonstrating the quality of the practices is fundamental to make the particular changes, which aid to develop and encounter the requirements of students and teachers. Objective: To determine the quality of medical-surgical training practice for the professional performance of Nursing. Methods: Quantitative approach, with a descriptive scope and cross-sectional design, an instrument composed by 22 questions that focused six dimensions was used, with a reliability of Cronbach's alpha of 0.99, on the survey directed to the students a simple random sample was used, and for teachers, a non-probabilistic sample was convenience. Results: The quality of the training practices from the perception of the students is good with 83.1% in relation to the accompaniment and help received by teachers. Likewise, teachers consider that the quality of the practices is good, according to the support received by the university, and the fulfillment of their responsibilities. Conclusions: The quality of the training performs within the surgical area is considered decent, both for those who obtain the practice and for those who teach it. Thus, certain factors are contemplated, such as the support of trainers and aid provided by the university.

    Complicaciones en pacientes diabéticos con covid-19

    Introduction: During the COVID-19 pandemic, the diabetic population has been considered vulnerable and at high risk as they are more prone to acquiring respiratory infections due to deterioration of the immune system and alteration of glucose levels. Objective: To describe the complications in diabetic patients with COVID-19. Methodology: meta-analysis of articles published until September 14, 2020 in databases such as: Medline, PubMed, Lilacs, Scielo, Redalyc, Elsiever, BVS and academic Google. The search was carried out in 4 phases; the first for search, the second for selection, the third for information extraction and the fourth for analysis. Results: The main complications of diabetic patients with COVID-19 are: severe respiratory distress syndromes and extrapulmonary systemic hyperinflammation syndrome, shock, vasoplegia, respiratory failure, cardiopulmonary collapse, severe pneumonia, myocarditis, acute kidney injury, pancreatic damage, this due to immunological and hypoglycemic mechanisms that lead to an increased risk of death. Conclusion: Patients with diabetes do present greater complications in COVID-19 pictures, since when there is inadequate glycemic control, there is a greater susceptibility to developing a severe septic picture due to a failure in the body's response to pathogens.

    “CONTROLES PRENATALES Y SU INFLUENCIA EN LA PRESENCIADE AMENAZAS DE PARTO PREMATURO EN MADRES ADOLESCENTES QUE ACUDEN AL CENTRO OBSTÉTRICO DEL HOSPITAL PROVINCIAL DOCENTE AMBATO EN EL PERÍODO ENERO – JUNIO 2012”

    La presente investigación considera a los controles prenatales como el pilar fundamental dentro de la prevención de diversas patologías que puedan afectar el desarrollo normal de un embarazo afectando la salud de la madre adolescente y del neonato; dentro de este ámbito. Se realizó un estudio descriptivo en los pacientes que acuden a centro obstétrico del Hospital Provincial General Docente Ambato, integradapor 50 pacientes, aplicando un enfoque cualitativo y cuantitativo. Se analiza el número de controles prenatales y su influencia en la presencia de amenaza de prematuros en madres adolescentes, mediante la aplicación de una encuesta y una guía de observación directa, obteniendo que el 68% de las investigadas, no se realizaron el número de controles prenatales adecuados durante su etapa de gestación. Se evidencia que la patología de Amenaza de Parto Prematuro es de origen multifactorial en la que interviene la edad materna, antecedentes de enfermedades maternas, el nivel de educación, procedencia, número de gestas, etc., que influyen en la aparición de esta patología. Se demostró que el hecho de que las madres adolescentes no se realicen un control prenatal optimo si influye en la aparición de Amenaza de Parto Prematuro. Ante estos resultados se propone un departamento de consejería materno – neonatal con una charla educativa y trípticos dirigidos hacia las madres adolescentes con la importancia de los controles prenatales a fin de capacitar, educar e incentivar a que acudan a realizarse los mismos, de manera oportuna para prevenir la aparición de patologías que impidan salvaguardar la vida del binomio madre- hijo en el servicio de centro obstétrico, con la aplicación de esta propuesta se beneficiará directamente al personal, pacientes, institución y comunidad.

    HOSPITALIZACIÓN PREVENIBLE EN ENFERMEDADES CRÓNICO DEGENERATIVAS: HIPERTENSIÓN ARTERIAL Y DIABETES

    Introduction: Chronic degenerative diseases are those that degrade physically and/or mentally, causing an imbalance that affects organs and tissues. Objective: Describe the importance of preventable hospitalization in chronic degenerative diseases, diabetes and high blood pressure. Method: Systematic review of literature and selection of articles using the PRISMA method. Results: 12 publications were chosen according to search keywords, inclusion and exclusion criteria established for selection. Conclusions: The application of a timely health system through Primary Health Care reduces rates of complications, hospitalizations and mortality of patients diagnosed with Arterial Hypertension and Diabetes Mellitus, through coverage, detection and monitoring of cases, therapeutic adherence, controls frequent interventions, education and self-care of the patient, provided by the nursing staff, fulfilling their role of promotion, prevention, cure and rehabilitation

    INSTRUMENTO PARA LA VALORACIÓN DE LA ACTUACIÓN DE LOS PROFESIONALES DE ENFERMERÍA EN LA ATENCIÓN PRIMARIA EN SALUD DE LA DIABETES MELLITUS

    Introduction: According to the WHO, diabetes is a chronic disease that occurs when the pancreas does not produce enough insulin or the body does not effectively use the insulin it produces. Objective: Design an instrument that allows for the assessment of nursing performance in Diabetes Mellitus in primary care. Method: Research with a quantitative approach, experimental, descriptive and transversal design. Development of an instrument to determine its reliability and content validity. The sample used for the evaluation of internal reliability was made up of 10 patients diagnosed with diabetes mellitus who are treated in the health centers of zone 3 belonging to the Pelileo Canton, Province of Tungurahua, who agreed to collaborate with the research, to check the understanding of the questions and the operation of the instrument. Internal consistency was determined through the calculation of Cronbach's Alpha coefficient. Results: Expert professionals in the subject of diabetes mellitus assessed the pertinence, relevance and clarity of the 20 items divided into 4 dimensions, qualifying them as applicable. The internal consistency of Cronbach's Alpha was 0.97; which demonstrates that the formulation of the items is reasonable, satisfactorily defined and is suitable in the presence of the questionnaire. Conclusions. The results obtained to evaluate the instrument and the application of the pilot test were adequate. This instrument is valid and reliable, which justifies its use in the research field. The instrument is easy and quick to apply, allowing us to know the perception of patients with Diabetes Mellitus about the care received by the nursing staff in the PHC.

    Introduction: Blood hypertension (HTA) is a disorder in which blood vessels persistently have high blood pressure and affects more than 1280 million adults in the world. Objective: Design and measure the reliability of an instrument to evaluate the performance of nursing professionals in primary health care: arterial hypertension Method: Quantitative approach, experimental design, descriptive and transversal. Development of an instrument to determine its reliability and validity of content. The sample used for the evaluation of internal reliability was composed of 10 patients diagnosed with arterial hypertension who are treated in health centers in zone 3 belonging to the province of Cotopaxi of Salcedo canton who agreed to collaborate with the investigation, check the comprehension of the questions and the operation of the instrument. Internal consistency was determined by calculating Cronbach’s Alpha coefficient. Results: Experts on the topic of arterial hypertension evaluated the relevance, relevance, and clarity of the 20 items divided into 4 dimensions, qualifying them as applicable. The internal consistency of Cronbach’s Alpha was 0.96, which shows that the formulation of the items is reasonable, satisfactorily defined and is adequate in the presence of the questionnaire. Conclusions. The results obtained to evaluate the instrument and the application of the pilot were adequate. This instrument is valid and reliable, which justifies its use in research.     Manejo de enfermería al paciente no oncológico en etapa paliativa

    Introduction: Chronic non-cancerous pathologies affect a lot of people, because they do not have an able curative therapy to slow down its progress. That is why it leads to the imminent death of the patient, and it is considered essential that it can benefit from the application of palliative care. Objective: To identify the nursing management of non-cancer patients in the palliative stage. Methodology: A quantitative, descriptive and field-research was carried out. The study population consisted of fifteen nurses, considering the inclusion and exclusion perspectives. To collect the information, it was necessary to make a survey of twenty closed questions, divided into four dimensions: Environmental Management, Emotional Management, Symptom Management and Likert-type Family Support, which was subjected to validity and reliability. Results: They showed high percentages in the management of the ambience and symptoms, nevertheless, there is evidence of inadequate emotional and family management by the staff. Conclusion: The lack of knowledge about the management of non-cancer patients and the discomfort, for being such a personal issue, that currently leads into a deficit in the management of these needs. They should focus on individuality, continuity and flexibility, considering the evolution of the disease, to have adequate management of symptoms, facilitate adaptation processes, emotional support and contribute to communication between the patient, the family and health personnel, respecting values and beliefs.

    La muerte cómo proceso natural en fase terminal

    Introduction: Nursing staff must acquire experience and knowledge about the physical and psychological management of cancer patients, considering death as the end of life, the management and acceptance of grief must be a priority, taking into account that death of patients indirectly affects health personnel on a psychological level. Objective: To identify the acceptance of death as a natural process in terminally ill patients, by Nursing staff. Methodology: The research was quantitative, field and descriptive, whose population was 24 nurses who work at the Hospital Solca Núcleo de Tungurahua, the survey was applied based on the questionnaire of attitudes towards death that addressed 6 dimensions. Results: 83.33% have thought of death as a possible fact, 66% are afraid of dying young and find it difficult to face death, 62% report that when they attend a terminal patient they have thought about the possibility of his death and it moves him to face his family, 29% see death as a passage to eternity and think that there is life after death and 33% show that death can be an exit to the load of the life. Conclusion: The death of a patient in the terminal phase is a scene that the nursing staff faces daily, accepting it provides an adequate management of grief and allows to provide care with empathy directed to the patient.

    Percepción y conocimiento de los internos rotativos sobre el formato Soapier en el registro enfermero

    Background: The use of SOAPIER data registers is part of the problem-oriented medical record data system, that explains the difficulties of patients. Objective: To determine the perception and knowledge of rotating interns about the SOAPIER data in the nursing register. Methods: Quantitative and descriptive approach, structured by 107 rotating interns who work in 4 hospitals in the zone 3 of Ecuador. The data collection done through the application of a survey that consisted of two dimensions, the first called perception structured by 6 items and the second knowledge that consist of 4 items, the consistency of the instrument is trough Cronbach's Alpha Coefficient which value was of 0.815. Results: Concerning the perception dimension, for 92.59% of rotating interns, the application of the SOAPIER data register is useful; for 74.07%, this registry permitted to emphasize on the interventions and evaluations performed in nursing; In addition, 51.85% consider it as a satisfactory tool. On the other hand, in the knowledge dimension, 51.9% of nursing interns have information about the SOAPIER format, its applicable, effective and unique; Conclusions: The SOAPIER format is described by being an significant instrument for the growth of rotating nursing interns, allowing the information to be consider in a logical, organized and ethical order, thus making it possible to provide quality healthcare.

    Síndrome de fragilidad en adultos mayores

    Introduction: Frailty syndrome is a condition associated with aging with a high incidence in older adults. Objective: To identify the frailty syndrome in older adults from the "Sagrado Corazón de Jesús" nursing home. Methods: Descriptive, cross-sectional and correlational study. The total study population was 60 patients, the instrument applied Frailty Tilburg Indicator (TFI) whose usefulness allows to measure the total frailty, as well as each domain of frailty, within which is: physical frailty with 8 items, cognitive frailty with 4 items, and social fragility with 3 items. Results: Physical frailty has a mean of 2 points, a maximum of 8 points with a mean of 6.8 points and σ=1.55. Psychological fragility shows a minimum of 0 points and a maximum of 4 points with a mean of 3.25 points and σ=0.79. Social fragility shows a minimum of 0 points, a maximum of 1 point with a mean of 0.9 points with σ=3.0 points. Conclusions: Frailty is a complex syndrome, which requires a special approach because it is a frequent disorder in the elderly, for which it is essential to carry out a rapid screening through the application of scales, to know the lifestyle, the risk factors, and the motor and mental skills that the elderly have in order to be able to interpret the management that the patient requires.
