    The purpose of organ transplantation is to preserve, improve and expand people's quality of life, it provides important social and economic benefits, it allows people to have a new opportunity for an active and productive life, the lack of donors causes a shortage in the availability of organs. that does not allow a solution to the global problem. 22.5% of the Ecuadorian population refuses to be donors. This problem is multifactorial; cultural, organizational, legal, economic, communicational, and training or educational aspects influence establishing the culture of donation. The objective of this article is to describe the current situation of donation and transplantation in Ecuador, with the policies and measures established so far and to establish the main challenges necessary to establish a culture of donation, in accordance with international recommendations and other major programs. success. A systematic review was carried out through a systematic electronic search of articles published from 2018 to July 2023 in the databases PubMed, Springer Scielo, ELSEVIER, Google Scholar, in addition to internationally verified information sources such as the World Health Organization. Health (WHO), Pan American Health Organization (PAHO), Ministry of Health of Ecuador (MSP). Considering that the barriers for donors are of multifactorial origin, it is concluded that active promotion through the media, education in schools and collaboration with public figures can increase community participation in organ donation.
