“Enfermedad renal crónica secundaria a litiasis coraliforme”
(Universidad Técnica de Ambato - Facultad de Ciencias de la Salud - Carrera de Medicina, 2018-05-01) Velástegui Villa, Orlando Israel; Guacho Guacho, Juan Sebastián Dr.
Renal lithiasis is a frequent pathology, so that approximately about 15% of the population of industrialized countries have a symptomatic episode before 70 years of age. Its etiology depends on the presence of several associated factors such as lifestyle, previous pathologies, hereditary, etc. The staghorn lithiasis is a variant of nephrolithiasis that has a recurrent urinary infection as a common predisposing factor; its repercussion will depend on the time and the evolution that this pathology presents. We present the case of a 56-year-old female patient with a history of uncontrolled arterial hypertension and recurrent urinary tract infections, who came to the Regional Teaching Hospital "Ambato" for presenting abdominal pain of weeks of evolution that is exacerbated in the last days before admission, accompanied by abdominal distension and respiratory distress that forced the patient previously attended a private doctor, where an ultrasound was performed that showed the presence of left hydronephrosis with non-specific right renal involvement at the examination. Again in the aforementioned health house, more complementary tests are performed where a considerable elevation of azote levels is evidenced, which indicates an important renal failure and which, given the evolution time, is categorized as a chronic kidney disease. It complements with advanced imaging tests that support the diagnosis of left hydronephrosis accompanied by the presence of staghorn lithiasis kidney, which is why hospital management was decided to stabilize the clinical picture and subsequent initiation in hemodialysis therapy.
