The vault has narrow acromiocoracoidea anatomical relationships with rotator cuff. Neer described the conflict in 1972 (impingement syndrome, impingement syndrome) between the anterior acromion and the rotator cuff (supraspinatus), from which the anterior acromioplasty described. This surgical procedure can decrease the compression subacromial. (1) It is the case of a female patient of 63 years of age, according to their history relates that started about a year ago showing a fall, after this incident goes to a center where empirical treatments are done without obtaining pain relief. After the patient went to the Hospital of the Ecuadorian Institute of Social Security Ambato feel treated in emergency where he was prescribed painkillers, but no pain relief after about a month she was treated by a general practitioner in the same nursing home and this transferred to Basic Private Tungurahua Hospital Order of the Hospital of the Ecuadorian Institute of Social Security Ambato to be treated by a specialist here the history in which the patient is seen with his fallen shoulder performed subsequently complementary tests requested, obtaining as a presumption diagnosed one acromioclavicular dislocation and impingement muscles of the rotator cuff. By getting this diagnosis decompression surgery for rotator cuff is planned. This surgery was ajar type wherein the total breakdown of the muscles of the rotator cuff was found for this double anchorage was used with good results in terms of pain relief, although complicating shoulder mobility and the absence of the acromion affect the biomechanics of it. After surgery the patient had to stay on medication for pain relief and especially to keep resting his shoulder, three months was sent to the area of Physical Therapy at the Hospital of the Ecuadorian Institute of Social Security Ambato.
