Item ENFOQUE DIAGNOSTICO DEL SÍNDROME DE EMBOLIA GRASA, UNA REVISIÓN BIBLIOGRÁFICA(Universidad Técnica de Ambato/ Facultad de Ciencias de Salud /Carrera de Medicina, 2024-06-10) Suárez Villacís, Bryan Eduardo; López Moya, Andrea GabrielaIntroduction: Fat embolism syndrome (SEG) is an ill-defined clinical condition attributed to the entry of fat emboli into the circulation. SEG occurs mainly because of orthopedic trauma, and less frequently to other non-traumati c conditions (pancreatitis, sickle cell anemia, osteomyelitis, diabetes m ellitus, fatty liver disease) or after an iatrogenic intervention (longterm corticosteroid therapy, liposuction). Objective: The objective of this work is to learn about fat embolism (GE) and FES, and to present a narrative review focused on its diagnosis, covering its definitions, causes, presentation and treatment. Materials and methods: Review and selection of scientific articles from different databases (PUBMED, Scopus and MedLine), in English. 26 articles (types) were included. Discussion: Despite the growing body of literature describing the occurrence of SEG, significant clinical uncertainty remains regarding its diagnosis, prevention, and treatment. Conclusions: A more detailed understanding of the pathogenesi s of this rare syndrome will facilitate specific strategies for prevention and treatment. More high-quality prospective clinical trials are needed to clarify the role of pharmacological treatment in SEG.Item Reporte de caso clínico de malformaciones genéticas no especificadas en el área de neonatología del hospital general docente Ambato(Universidad Técnica de Ambato/Facultad de Ciencias de la Salud/Carrera de Enfermería, 2022-03-01) Llanga Santana, Joselyn Monserrath.; Velasco Acurio, Evelin Fernanda Lic. Mg.Objective: To identify unspecified congenital malformations through the review of the clinical history of a patient in the Neonatology area of the Hospital General Docente Ambato. The research was framed in the descriptive type, information was collected through the review of the clinical history obtained at the Hospital General Docente Ambato. Case description: a newborn girl was analyzed in the Pelileo city, province of Tungurahua, who two hours after delivery presented respiratory difficulty and was admitted to the Hospital General Docente Ambato, where after being evaluated by the medical staff she was diagnosed with unspecified congenital malformations. She remained in the hospital for 44 days; however, despite her deteriorated state of health she was removed due to the lack of commitment of the parents with the treatment indicated by the health staff. Esp.The present research work aims to determine the influence of the overload syndrome on the quality of life of the caregiver of patients with disabilities in the first level of care. The care environment that is provided to a person with a disability demands great responsibility with total use of time to provide care to the dependent patient and with it the consequent work overload that in turn will produce negative effects on the physical, emotional, social and quality of life of the caregiver. The study methodology is quantitative, field, bibliographic and correlational. The study population is 50 caregivers of patients with disabilities belonging to the Pilahuín parish. The research techniques and instruments were the survey based on the WHO SF-36 questionnaire to establish the level of quality of life and the Zarit Test was used to determine which population of caregivers suffers from overload syndrome. The results determined that 76% present a medium- high level of quality of life, also 52% present mild overload, while 32% do not suffer from overload and 16% have severe overload. It is concluded that overload syndrome influences the quality of life of caregivers of patients with disabilities, there is asignificant relationship between the variables. The proposal is the design of a health promotion strategy aimed at caregivers of patients with disabilities for the prevention ofoverload syndromeItem “Síndrome de intestino corto como complicación de perforación intestinal y hernia inguinal”(Universidad Técnica de Ambato - Facultad de Ciencias de la Salud - Carrera de Medicina, 2018-05-01) Herrera Lescano, Roger Vinicio; Navarrete Álvarez, Luis Edmundo Dr. Esp.We present the case of a female patient, adult, subjected to left inguinal herniorrhaphy that triggers short bowel syndrome. The short bowel syndrome used to refer to malabsorption, resulting from previous resections of the intestine (acquired), or its absence due to malformations (congenital). Objectives: To describe the clinical picture, risk factors, diagnostic and therapeutic procedures, and identify the critical points of the short bowel syndrome. Research method: Bibliographic review in scientific texts and articles; review of clinical history and direct interview with the patient. Conclusion: The short bowel syndrome is caused by a decrease in the functional surface of the small intestine causing clinical, metabolic and nutritional alterations. This abnormality responds to various etiologies, being its main complication states of malnutrition. The management is focused on achieving a nutritional balance that compensates the basic requirements. Recommendations: • Carry out an exhaustive control of electrolyte values. • It is recommended to start enteral nutrition early, in the phase of adaptation of the SIC, where the absorptive capacity is increased.Item Síndrome de West a Causa de Isquemia Cerebral Antes del Nacimiento(Universidad Técnica de Ambato-Facultad de Ciencias de la Salud-Carrera Terapia Física, 2016-11-01) Sánchez Carrera, Elizabeth Jazmina; Jerez Camino, Carlos Milton Md.This research documents in detail the analysis of the clinical case with the theme: West syndrome caused by cerebral ischemia before the birth. In a 10 years old male patient. The West syndrome is a pathology that is presents because of an encephalopathy hypoxic-ischemic and is usually found accompanied by brain palsy, has an incidence of around 1 by 4,000 to 6,000 infants living with predominance in the sex male, its main features are: epileptic spasms, delay of the psychomotor development and hipsarritmic electroencephalogram. This is a clinical case with complications before and during birth which will identify risk factors suchas: abdominal, lack of a good trauma monitoring service is health before and during the labor and delivery and late medical diagnosis; THe patient has received physiotherapist treatment starting from the third year of life with little advance, by what is proposes a physiotherapist treatment based in the "guide of treatment integral of the spasticity" of where is chose the Bobath method as one of them main approaches therapeutic in the present work.Item Influencia de la Terapia de Integración Sensorial en el Tratamiento de la Hipotonía en Niños con Síndrome de Down(Universidad Técnica de Ambato-Facultad de Ciencias de la Salud-Carrera Terapia Física, 2016-11-01) Romero Rivera, Marlon Alexis; Campos Moposita, Ángela Priscila Lcda.The aim of the following investigation project is to analyze the sensory integration therapy influence over the hypotonia treatment in children with Down syndrome, to verify the muscles development benefits important in the psychomotricity. Where the hypotonia was evaluated through an observation form based on psychomotor activities such as: seating stability, crawling, standing position, marching and the fine motor control. The population for this study were 30 children from the Elementary School “Ambato Specialized” in the age from 1 to 4 years with hypotonia caused by Down syndrome. Furthermore, to prove the hypotonia in which was achieved the muscle tone decreased that presented the children. The investigation project was based on the qualitative and quantitative approach since the data was collected through the observation form, based on fine and gross motor activities and punctuated in the Campbell Scale. In the beginning that was obtained that 37% of children with severe hypotonia and end only 7% of them.Item Síndrome de fragilidad en el adulto mayor y su repercusión en la salud familiar, Parroquia Cunchibamba 2015 - 2016(Universidad Técnica de Ambato-Facultad de Ciencias de la Salud-Dirección de Posgrado, 2016-11-01) Garcés Jerez, Katherine Elizabeth Md.; Linares Valdés Dra Esp, HortensiaIntroduction: Ecuador presents, currently, an accelerated aging process, a phenomenon that causes health problems among older adults, including the fragility that is a relatively new concept in geriatrics. The frailty syndrome is a pre disabled state, a state of vulnerability, or is detected and thus can address the situation to delay adverse events or overlooked and the elderly can cross the thin line separates fragility of disability. Objective: To identify frailty syndrome in the elderly and their impact on family health. Methods: Cross-sectional, descriptive, observational, with mixed approach of a total population of 200 adults over 65 years of sectors served by postgradistas of Family and Community Medicine, parish Cunchibamba, Study 2015/2016 period. Results: The frequency of frailty in the elderly was 163 (81.5%), the age range of 71-90 years predominated with 48.5%, 59.6% were women. It was classified according to the degree of functional impairment, mild weakness in 71 (43.5%), moderate 57 (34.9%), severe 35 (21.4%). Among the sociodemographic characteristics, marital status married 87 (53.4%), the level of instruction illiterate - incomplete primary 102 (62.5%) and employee work status 86 (52.8%) had significant percentages in different degrees of fragility. factors associated with the frail elderly, polypharmacy, hospitalization, depression, cognitive level and comorbidities with statistically significant results were identified. The family impact before the event frail old man, was coded by level of impairment, mild 47 (29.0%), moderate 49 (30.0%) and severe 21 (13.0%). Conclusions: A high level of fragility was identified in the elderly, with moderate impact on family health, mainly affecting the economic area, without underestimating the socio-psychological areas and family functioning.Item Miocardiopatía Hipertrófica Apical que Simula un Síndrome Coronario Agudo(Universidad Técnica de Ambato-Facultad de Ciencias de la Salud-Carrera Medicina, 2016-10-01) Arce Pérez, Yanael Marcelo; Mora Brito, Edgar Vicente Dr. Esp.Hypertrophic cardiomyopathy is a myocardial disease that is caused by genetic alterations in genes encoding different sarcomeric proteins, is presented as a thickening of the ventricular wall variable location, and has a wide clinical spectrum of expression ranging from asymptomatic carriers to those which they present sudden death as the first manifestation of the disease. Apical hypertrophic cardiomyopathy is a rare variant, with circumscribed hypertrophy to ventricular apex. It has well-established electrocardiographic characteristics; but it can mimic other diseases. Starting from this premise, the analysis of this case report is to establish guidelines therapeutic diagnosis, and mention of the major complications for the proper management of apical hypertrophic cardiomyopathy. The case of a male patient, 49 years old, diabetic, with a family history of sudden death. Go to the emergency with severe pain in the left lower limb, after physical exertion and referred occasional palpitations, he has never presented dyspnea, angina or syncope. Examination revealed: rhythmic heart sounds of low intensity, and positive signs Lasègue Bragard. The electrocardiogram reveals deep negative T waves in precordial leads V3-V6. Suspecting acute coronary syndrome admitted with antithrombotic therapy. The obtained negative cardiac enzymes, and once passed the lower limb painful picture because ciatalgia the patient undergoes a stress test without registering electrocardiographic changes. Having no conclusive diagnosis, magnetic resonance imaging, which identifies severe hypertrophy in the ventricular apex is requested. Antithrombotic drugs are suspended and beta blockers start. The final diagnosis was apical hypertrophic cardiomyopathy and sciatica. Patient evolved properly.Item Incidencia del Síndrome de Burnout en el Desempeño Laboral de los Fisioterapeutas que Laboran en las Unidades de Salud Pública del Cantón Ambato(Universidad Técnica de Ambato-Facultad de Ciencias de la Salud-Carrera Medicina, 2016-10-01) Ramos Robalino, Belén Isabel; Naranjo Reino, María Teresa Lcda.This research was conducted in the institutions of public health service of the Canton Ambato who have the area of Physical Medicine and Rehabilitation. With the main objective of the research it is: To determine the incidence of Burnout syndrome in physiotherapists who work in public health units of Ambato Canton. This study was observational descriptive transversal, the universe chosen was 29 Physiotherapists of Ambato Canton and data collection was performed once with the survey of personal and occupational factors and questionnaire MIB Maslach Burnout Inventory, in order to determine the Burnout Syndrome incidence in Physiotherapists which it turned out to be 55.17% of 100% of the study population results found that females with Burnout is 34.5%, age and years of service given much significance in the study since 60 % of the population has more than 10 years of service and aged 41 to 55 or more and obtained satisfaction survey care service perceived health user thus conclude that the professionals had Burnout do not provide quality care and generate unequal treatment and users felt dissatisfied with the attention with a percentage of 42.4% of users served by professional participants. So this study seeks to foster in the institutions including a plan of prevention and treatment to health professionals at risk or suffering from work-related stress syndrome.Item Crisis Convulsivas por Hiponatremia secundaria a Síndrome de Sheehan(Universidad Técnica de Ambato-Facultad de Ciencias de la Salud-Carrera Medicina, 2016-10-01) López Mayorga, Oscar Ricardo; Dr. Guarnizo Briceño, José AbdónThis study analyzes the clinical case of a patient with seizures per secondary hyponatremia syndrome Sheehan, one of the pathologies associated with patients with postpartum bleeding is one of the rare complications of these diseases but with early treatment and shall not exceed their proper conditions. A case of a patient with gynecological history not to have menstrual cycles after his only feat 8 years is presented, comes because of vomiting in copious amounts of food content then proceed to be hematological parameters, which is accompanied by tonic-clonic seizures , impaired level of consciousness and disorientation, so is brought to this house health. The patient is referred to the internal medicine department where it is valued, additional tests are performed and Sheehan Syndrome, after which it is treated side effects and the disease is diagnosed.
