REVISIÓN SISTEMÁTICA: ACTUALIZACIÓN DE TERAPIA INTRAARTICULAR EN LAS PATOLOGÍAS TRAUMATOLÓGICAS
(2025-06-16) Solis Cadena, Dayana Monserrath; Marizande Lozada, María Fernanda; Universidad Técnica de Ambato / Facultad de Ciencias de Salud / Carrera de Medicina
Introduction: Traumatologic pathologies such as osteoarthritis (OA) affect a large part of the world's population, especially in developing countries, with predisposing factors such as sedentary lifestyle, obesity and trauma. The prevalence of OA increases with age, being more frequent in people over 60 years of age. In Latin America, the prevalence varies, being 7.4% in Ecuador. Short-term treatment aims to relieve pain and improve function, while long-term treatment attempts to slow disease progression. Intra-articular therapies, such as corticosteroids and platelet-rich plasma, are common options, but their long-term effectiveness is uncertain. Objective: The purpose of this paper is to evaluate recent evidence on the effectiveness of intra-articular therapies in the management of traumatologic pathologies. Materials and Methods: A comprehensive search was conducted in databases such as PubMed, Cochrane Library, and Scopus, including studies in English and Spanish published between 2019 and 2024. 21 studies were selected that met criteria such as use of intraarticular therapies (PRP, HA, corticosteroids, combinations). Results: Plateletrich plasma (PRP) is more effective than hyaluronic acid (HA) and corticosteroids in relieving pain and improving joint function in the long term. HA improves pain and function, but its effect is less than PRP. Corticosteroids offer temporary relief, but with risks of side effects. The combination of PRP and HA shows superior benefits. Mesenchymal stem cells (MSC) promise regenerative effects, but require further studies to confirm their long-term safety and efficacy. Discussion: Intra-articular therapies are a key alternative for patients with trauma pathologies seeking non-surgical options. Although PRP emerges as the most promising treatment, variability in methods and populations studied limits the generalizability of findings. Furthermore, combining therapies might offer a new avenue of management but requires further robust research. Conclusions: PRP stands out as the most effective therapy for pain relief and joint function improvement in trauma pathologies, outperforming HA and corticosteroids. Combinations such as PRP + HA are promising, while mesenchymal stem cells offer an innovative option for the future. However, further studies are needed to confirm these results and ensure long-term safety.

TERAPIA CON CÉLULAS MADRE MESENQUIMALES PARA REGENERAR Y REPARAR EL CARTÍLAGO DAÑADO EN LA ARTROSIS
(2025-06-13) Guanoluisa Andagua, Erika Elizabeth; Estrada Zamora, Esmeralda Maricela; Universidad Técnica de Ambato / Facultad de Ciencias de Salud / Carrera de Medicina
Osteoarthritis (OA) is a highly prevalent, multifactorial pathology that mainly affects older adults. It is projected that by 2050 there will be 1.6 billion people in this age group, which makes the development of a useful therapy for OA essential in its development. OA mainly affects cartilage, damaging and weakening it, causing chronic pain, inflammation and a loss of quality of life for the patient who suffers from it. This study aims to demonstrate the benefits of new therapies focused on cartilage regeneration, such as regenerative therapy with mesenchymal cells (MSC) with the aim of being considered as an option in our country to treat these patients. This is a systematic review of the bibliography, of a qualitative nature, of books and scientific publications that have been published during the last 5 years in English and Spanish on therapy with mesenchymal cells to regenerate and repair damaged cartilage in osteoarthritis. databases such as PubMed, Scopus, Web of Science and ScieLO. MSCs are an innovative therapeutic that still require more studies in order to provide the greatest therapeutic benefit in OA. The most effective MSCs are autologous ones since they have lower rejection rates and greater regenerative benefit. The best route of administration is localized as it specifically promotes cellular benefits in the affected tissue.

Aplicación de un Protocolo de Ejercicios con Facilitación Neuromuscular Propioceptiva para mejorar rango y fuerza muscular en adultos mayores con dolor de rodillas.
(2021-07-01) Manzano Vinueza, Gabriela Cristina Lcda. Ft.; Latta Sánchez, María Augusta Lcda. Ft.Mg
Knee pain is one of the main problems for the population over 65 years old and is related to degenerative diseases such as osteoarthritis. Exercises with the Proprioceptive Neuromuscular Facilitation technique (PNF) have been suggested as an alternative rehabilitation treatment to reduce the impacts of this frequent symptom. The purpose of this project was to apply an exercise protocol with the Proprioceptive Neuromuscular Facilitation technique to improve range of motion and muscle strength in older adults with knee pain. Twenty older adults with an age range between 65 and 80 years from the Atahualpa parish participated, all of them performed the protocol for 12 weeks with a frequency of 1 session per week. The patients were evaluated before and after the intervention using the numerical pain rating scale, goniometric test and Daniels test. The resistance, difficulty and number of repetitions applied in the lower limb diagonals were gradually increased. The SPSS program was used for statistical analysis. The results obtained confirm the hypothesis that the parameters tested: pain, joint mobility and muscle strength improved after the use of PNF techniques. It is concluded that the three specific techniques: Rhythmic Initiation, Dynamic Inversions and Contraction-Relaxation belonging to the concept of Proprioceptive Neuromuscular Facilitation applied in an exercise protocol increase physical capacities such as mobility and muscle strength in the older adult population aged 65 to 80 years. Future and more extensive research is necessary considering the variables or limitations that may have intervened in this study.
